NEW YORK — Thanks to new funds, EMS, medical and hospital services will now be able to expand their patient care.
The effort is part of New York State’s $8 billion investment to advance care for patients receiving Medicaid as well as help decrease hospital readmissions.
The funds from the Montefiore Hudson Valley Collaborative will help Rockland Paramedic Services, who were awarded more than $350,000, to train paramedics in order to expand their role to post-hospital visits. The paramedics will visit the patients in their homes to help with discharge and medication instructions.
Additionally, Nyack Hospital, who was awarded $275,000, will be the first New York hospital to use the Twiage app which enables responders to send live data about patients through videos, photos, voice memos and text messages.
Maternal Infant Services Network, who was awarded $250,000, will use the money to help connect teens to primary care and help with lifelong healthy habits.
