The Philadelphia Fire Department is hard at work hiring paramedics, Commissioner Lloyd Ayers said yesterday.

But City Council members considering his agency's annual budget were clearly impatient with the pace, since understaffing has been a significant problem.

Mayor Nutter budgeted $3.9 million more for the fiscal year that starts July 1 to help the department hire more paramedics and put more medic units on city streets ... Full Story

The Ambulance Driver's Perspective
The Two Most Important Words in an EMS Protocol
Allow me to introduce you to the two most important words in an EMS protocol: May and consider — please take a bow.

You two words are what make clinical decision-making possible. Without you, we'd all be doing procedures of questionable benefit to patients who may or may not need them, all because it's in the protocol.

Oh, wait a minute. We are doing unnecessary things just because they're in the protocol ... Full Column

Dr. Bledsoe as usual highlights some excellent points. It appears that 99 of the MAST use "back in the day" were not helpful and probably harmful. The MAST is a tool. Like any tool there are circumstances when it should not be used. The MAST is a very specialized tool, which does still have at least one indication: fractured pelvis.

EMS1 Forums Post of the Week
"We often do prehospital blood draws. I find this decreases the patient's wait time in the ER, as well as alleviates the nurse's duties (they usually appreciate this, especially on busy days). Blood draws are easy to do, and can often have a high impact on the patient's overall care. I try to draw labs during any call where I'm going to administer a medication so that the hospital will have pre-intervention bloods."
