Investigators probe 'overlooked'

Alive, mistaken for dead


Ambulances descend on the scene of a car crash in Texas where a woman died from injuries after being presumed dead and left untreated for two hours. An investigation is pending. Full coverage (Photo courtesy of WOAI.com)

Authorities launched investigations Monday to determine whether rescuers properly assessed an accident scene where a 72-year-old woman was left inside a crumpled car that was towed to a police impound lot. A preliminary report said the woman died within minutes of the crash.

''Failure to dialyze led to hyperkalemia. A previous EKG would have shown an increasingly widening QRS. The QRS eventually became so wide that it had deteriorated into this. I'm surprised the patient isn't experiencing chest pain. He needs calcium immediately to help counterbalance the high potassium. Oh, and he needs dialysis, too.''
